Copy of ID document for the father / Guardian 1

Copy of ID document for the mother / Guardian 2

Immunisation / Clinic Card (Copy of medical aid card if applicable)

Letter of Employment of the PAYER / 3 month’s Payslip

Proof of Residence / Affidavit

Latest School / Nursery School Report
